The system

Conversation, examination, treatment. And a debrief after every case.

Students stand before the patient as they would in a clinic or on a ward. Every decision has a consequence, and every one of them stays for the debrief.

The 3D examination: Penka, 71, in the hospital bed, the instruments on the right and the examination findings.
The start of a ward session: Nadezhda, 69, with an allergic reaction after an antibiotic, the monitor and the conversation on one screen.

Conversation

Hello, Maria, come in. What brings you here today?
100. Hello, doctor. For two weeks I’ve had a headache in the mornings, at the back of my head. I’m worried because I measured my blood pressure at home with my husband’s monitor and it was 165 over 100.
Tell me about the headache. What does the pain feel like, and when does it go away?
It’s more of a dull ache or heaviness, not throbbing. It goes away after an hour or two.
How exactly did you measure it at home?
100. Twice in the morning, sitting down, with my husband’s monitor, the cuff above the elbow. Both times it was about 165 over 100.
Any problems with your vision, nausea, chest pain or shortness of breath?
No nausea, no vomiting, no problems with my vision. No tightness in my chest, and I’m not short of breath when I walk.
Do you smoke? And do you drink alcohol?
I smoke about ten cigarettes a day, for twenty years now. Nothing else comes to mind, doctor.
How do you eat, do you like salty food? Do you get enough exercise?
I don’t exercise much, I’m an accountant and sit at a computer all day. I like my food a bit salty.
Does anyone in your family have high blood pressure, or had a stroke or a heart attack?
My mother had a stroke when she was sixty-two. I think she had problems with her blood pressure too.
Do you take any medicines, for example the pill or painkillers?
Nothing regularly, only paracetamol or ibuprofen now and then for the headache. No pill and no hormones.
How do you sleep? Have you been under pressure lately?
I sleep six or seven hours. Work is stressful, the annual reports are coming up.
Maria, your blood pressure is high here too, 168 over 102. We need to get it under control, but it isn’t an emergency.
Thank you, doctor, what do we need to do now?
I’ll give you a referral for blood and urine tests, the nurse will do an ECG and we’ll start treatment. Try to stop smoking and eat less salt. I’ll see you again in two weeks.
